Volunteer forms for Lyon County Schools

Kentucky State Law passed that both forms below must be completed and approved to volunteer or attend a field trip. Both forms must be complete and you should receive an email of acceptance BEFORE you can volunteer or go on a field trip. It usually takes 4-6 weeks to be approved or denied. 

Volunteer Registration Form 

Fill out the form and then complete the Child Abuse/Neglect Form to be eligible to volunteer.

Child Abuse/Neglect Form

Fill out the form and submit to the Cabinet for Health and Family Resources. Required: fill out information, have someone to sign and witness the document, provide a copy of driver's license, social security card or birth certificate, and send a $10 check or money order made payable to the Kentucky State Treasurer.
