Student Registration Instructions

Student registration forms can be found in the link to the left under Forms and Documents then Beginning of Year Forms or Click Here then Beginning of Year Forms.


K, 6th and 9th grade students complete the Household Form and Student Information Forms.  Students in all other grades will need the Census Verification Form completed.  The Census Verification will be given out at your child’s school.  All students must complete the Medical Forms, Publication Consent, Consent for Minors, Military Connection, and Household Income Form.  The FERPA Opt-Out Form only needs to be completed if consent is not given to release Directory Information.


Brand new students to our school district will need to complete all forms for enrollment.  For additional enrollment requirements, please refer to the Informative Documents under Parent Resources.
