Volunteer forms for Lyon County Schools

Required forms that all volunteers and/or parents wanting to attend school trips must complete. 

Due to the COVID-19 pandemic, Lyon County Schools will limit volunteer opportunities unless they are absolutely essential. We will keep you posted, if this rule changes.

    Child must be 3 or 4 by August 1, 2021

    • 4 year old students whose family meets poverty guidelines according to their valid annual income are eligible to attend at no cost to parents. 
    • 3 and 4 year old children meeting the KERA disability guidelines are eligible to attend regardless of income.
    • If you do not meet these guidelines you may be asked to pay tuition.
    • Transportation may be provided

    Please bring the following documentation for registration:

    • Proof of residency (Driver’s License)
    • Custody documentation (If applies)
    • State issued birth certificate 
    • Proof of income
    • Social Security card (LCPS only)
    • Medical card (HS only)
    • Immunization record
    • Current physical
    • Eye exam due by January 1, 2022

    The third Friday of each month, after 3:00 P.M., routine pesticide treatments will be performed in all buildings.  Services are done by HomePro Advantage using Lambda Cyhalothrin (9.7%) with a pressure sprayer to classrooms, offices, hallways, restrooms and utility areas. Targeted pests are spiders, ants, beetles, etc. MSDS information is available upon request.  If your child has any allergies to such treatments, please call Rob Richey, Safety Director, at (270) 388-9715, ext. 413.  His office is located at 217 Jenkins Road, Eddyville, Kentucky.
